Factors regulating ventricular rates during atrial flutter and fibrillation in pre-excitation (Wolff-Parkinson-White) syndrome.

چکیده
Premature atrial stimulation and His bundle recordings were performed in 6 patients with pre-excitation (WPW) syndrome. The ventricular rates during bursts of atrialflutter and atrialfibrillation triggered by the earliest impulses depended on the functional properties of the accessory pathway. In 3 patients in whom the effective refractory period of the accessory pathway was longer than that of the normal AV pathway the ventricular rates did not exceed 2IO/min; the majority of the supraventricular impulses reached the ventricles through the normal AV pathway. In 3 other patients in whom the effective refractory period of the accessory pathway was very short (less than 220 msec) the ventricular rates were as high as 3IO/min; the majority of the impulses reached the ventricles through the accessory pathway. Concealed AV conduction occurred in both pathways producing RR pauses well in excess of the duration of the corresponding effective refractory periods. These tachyarrhythmias were due to stimulation during the atrial vulnerable period, not to a reciprocating mechanism involving two separate anatomical AV communications. Intracardiac recordings were necessary to evaluate the AV conduction patterns as well as the nature of the wide QRS complexes seen during the tachyarrhythmias.

Atrial flutter with 1:1 conduction in undiagnosed Wolff-Parkinson-White syndrome.
BACKGROUND Atrial flutter with 1:1 atrioventricular conduction via an accessory pathway is an uncommon presentation of Wolff-Parkinson-White syndrome not previously reported in the emergency medicine literature. Wolff-Parkinson-White syndrome, a form of ventricular preexcitation sometimes initially seen and diagnosed in the emergency department (ED), can present with varied tachydysrhythmias fo...
متن کاملElectrophysiological assessment of amiodarone in treatment of resistant supraventricular arrhythmias.
Oral amiodarone has been used to treat 21 patients with various supraventricular arrhythmias; 13 had Wolff-Parkinson-White syndrome, which was complicated by atrial fibrillation and re-entry atrioventricular tachycardia in four, and re-entry tachycardia alone in the other nine. The remaining eight patients had paroxysmal atrial fibrillation or flutter without pre-excitation. All were refractory...
متن کاملVentricular fibrillation. A possible mechanism of sudden death in patients and Wolff-Parkinson-White syndrome.
A case of Wolff-Parkinson-White syndrome associated with ventricular fibrillation is presented. The control of recurrent ventricular fibrillation with large doses of digitalis and other antiarrhythmic drugs, including propranolol, lidocaine, procainamide, and quinidine, is discussed. As far as we can determine, this is the first human case in which the precise onset of ventricular fibrillation ...
متن کاملWolff-Parkinson-White syndrome: atrial fibrillation as the presenting arrhythmia.
Atrial fibrillation was identified as the initial arrhythmia complicating the Wolff-Parkinson-White syndrome in ten (9%) of 108 patients. Despite initially rapid ventricular responses in seven, long term survival and control of arrhythmia were excellent on medical treatment. Whereas symptom free patients with the pre-excitation syndrome who have additional underlying disease predisposing to atr...
متن کاملAcute effect of disopyramide on atrial fibrillation in the Wolff-Parkinson-White syndrome.
Disopyramide was administered intravenously to 54 patients during atrial fibrillation and predominantly pre-excited QRS configuration at the time of electrophysiologic study. All patients had Wolff-Parkinson-White syndrome and no patient had coexistent heart disease. The drug was given during sustained atrial fibrillation (n = 45) or during sinus rhythm before induction of atrial fibrillation f...
